With P0191, prove the circuit before you condemn the part named in the scanner text. P0191 means the fuel-rail pressure sensor signal is implausible relative to commanded pressure or other fuel-system inputs. On common-rail diesels a noisy or biased rail-pressure reading destabilizes injection timing and quantity, producing surge, hard start, or derate. Scope the sensor supply, ground, and signal for dropouts; compare commanded versus actual rail pressure during a snap throttle; and rule out actual pressure problems (P0087/P0088) before replacing the sensor. Vibration-induced connector fretting on the rail harness is common on highway trucks. Discriminate among failing rail-pressure sensor with biased or noisy output, poor ground or 5 V reference shared with other sensors, and chafed rail harness from engine vibration using voltage drop and connector integrity. Verify by scope 5 V, ground, and signal while wiggling the rail connector, then compare commanded vs actual rail pressure during snap tests; replace common-rail fuel pressure sensor only when the wiring path checks out clean.
