P0420 rarely strands you, but a failed Bank 1 catalyst can fail emissions and overheat the exhaust on hard grades. Diagnose within a few drive cycles—especially before an inspection—and fix mixture/misfire roots before you authorize a converter.

"Efficiency below threshold" is judged from oxygen-sensor waveforms—not a direct count of pollutants at the tailpipe. The ECM compares Bank 1 Sensor 1 (upstream of the catalyst) to Bank 1 Sensor 2 (downstream). A healthy three-way brick stores and releases oxygen so the rear signal stays relatively flat while the front swings rich/lean; when that oxygen storage collapses, the rear starts mimicking the front's fast switching—and the monitor fails. The fail point (often cited near ~95% efficiency) is an OEM calibration for that powertrain, not a universal number; a factory reflash can sometimes clear a false P0420 if the software limit was set too tight. Bank 1 is the cylinder bank that includes cylinder 1 (on a V-engine the other bank uses P0430).

P0420 means the ECM judged Bank 1 three-way catalyst efficiency below threshold by comparing upstream (Bank 1 Sensor 1) and downstream (Bank 1 Sensor 2) oxygen-sensor activity. A healthy catalytic converter stores and releases oxygen as the mixture swings; that oxygen storage flattens the rear O2 waveform once the brick reaches light-off. When storage collapses—from age, poisoning, or damage—the rear sensor begins to switch almost as aggressively as the front, and the catalyst monitor fails. Drivers usually notice a check-engine light and inspection readiness problems long before a hard limp; some smell sulfur under sustained load or feel a mild top-end flat spot if the matrix starts restricting flow.

Rank causes by how often shops actually find them. Most often the Bank 1 converter itself is aged out after high mileage or years of short trips that never fully light the brick. Common next are exhaust leaks ahead of the rear sensor boss (false air makes the rear look “active”), a lazy or contaminated downstream O2, and oil or coolant ash coating the washcoat. Less common but important: chronic rich, lean, or misfire history that overheated or chemically poisoned an otherwise good converter—fix fuel trim and combustion first or the new cat will fail the same monitor.

Diagnosis starts hot. Bring coolant and catalyst temperature into the OEM enable window, then graph front and rear O2 (or AFR and rear O2 on wideband platforms) at a steady ~2000 rpm with closed-loop fuel control. A good rear signal stays relatively flat while the front switches; a rear that mirrors the front is the classic P0420 signature. Pull freeze frame for load, STFT/LTFT, and rear O2 voltage at the set. Smoke-test or listen for exhaust leaks from the head flange through the converter to the Sensor 2 boss. Clear misfire (P0300-family), fuel-trim (P0171/P0172-style), and heater codes before any oxygen-storage or delta-temperature test. Use Mode $06 catalyst test results and OEM oxygen-storage or inlet/outlet delta-T procedures when available—do not condemn the brick on a cold, short-trip drive cycle.

Replace the Bank 1 catalytic converter only after leaks, mixture, misfire, and rear-sensor response check out and storage/delta-T still fail. Where CARB-compliant parts are required, install a CARB-legal unit—not a universal “will-fit” shell. Dig elsewhere when a brand-new rear O2 suddenly “fixes” the code (sensor was the liar), when STFT is pinned rich or lean, or when an exhaust leak is audible at the Sensor 2 bung. System context: this is an emissions-efficiency judgment, not a wheel-speed or ABS path. Upstream O2 and fuel control protect the brick; downstream O2 mostly watches it.
